docs(data-integration): title every page with its USCDI data class - #55

The column heads "USCDI v3.1 data class"; row 12 held element names. Procedures is the class, as row 11 already says.
Procedures holds Procedures, SDOH Interventions and Reason for Referral, so service_requests belongs on a Procedures page. Conditions is Problems. Old URL redirects.

Mirrors the USCDI v3.1 hierarchy: one page per data class, one section per dataset inside it.

devices.mdis "Unique Device Identifiers" against the class name "Unique Device Identifier(s) for a Patient's Implantable Device(s)" — the short form reads better in the nav and matches the spec's own file index.
