docs(data-integration): add the procedures dataset - #56

The class holds three elements and the page covered two; Procedures itself had no section. Adds `service_request_id` for Procedure.basedOn, a USCDI element the spec has no column for.
Procedure.reasonCode binds to procedure-reason, not the Condition codes ServiceRequest uses. Pinned to 4.0.1 — unversioned resolves to R5 draft.
