## 2026-08-30 - Memoization of deterministic regex in context.py
**Learning:** `_compiled_context_lexicon` in `openmed.clinical.context` recompiles multiple regexes from lexicons on every call. This becomes a major bottleneck since `scan_context_cues`, `resolve_temporality`, `resolve_uncertainty`, and `resolve_negation` call it. Caching with `@functools.lru_cache` reduces scanning time by more than 70%.
**Action:** When working on NLP/text-processing, always memoize deterministic regex compilations and lexicon generation.
