Describe the bug
When a user submits the request form and is redirected to Aeon the form on Finding Aid should clear all data but it doesn't. The "cart"/form data is cleared if moving between finding aids as expected.
The form will allow you to submit the request again but the Aeon side will give the user an error "This form has already been submitted. Duplicate request not created."
There should also be a notice (text and/or external link icon) that upon "Submit Request" the user is going to be redirected to another site and that site opens in a new tab.
To Reproduce
Steps to reproduce the behavior:
- Go to any finding aid (ex. https://exploreuk.uky.edu/fa/findingaid/?id=xt75736m0k6d)
- Add any item to "cart"
- Fill out form
- Click "Submit Request" button.
- Return to same finding aid.
- See form still filled out
Expected behavior
Once a user submits the form, the form data should be cleared from the form.
