docs: organize operational records and evidence

Summary

• move active operational records from the repository root to docs/operations/
• move smoke-QA and pilot evidence from the root to docs/evidence/
• update navigation, checked paths, verifier paths, benchmark references, and CODEOWNERS

Scope

No content, governance semantics, production status, or evidence outcomes changed. Moves preserve Git history; root file count drops from 42 to 31.
