feat(claude): ask before launching a Fable subagent - #43

Fable subagents tend to spin out of control and burn tokens, so a launch with model: fable (or any claude-fable-* id) now asks for explicit confirmation via a new agent-guard PreToolUse hook, instead of slipping through under auto mode.
