ci: cancel stale OpenTofu PR runs

Summary

• add workflow-level concurrency keyed by workflow and pull request number
• cancel superseded runs, including runs replaced by Terraform-docs autofix commits
• use github.ref as the fallback key for push events

Testing

• verified the branch changes only add the requested concurrency block
