The narrative is the payload: US Core makes text.div mandatory, so a row without it cannot become a CarePlan. The assess-plan category and narrative status are fixed by the profile, so Payerbox sets both.

care_planspage → US Core CarePlan, covering the Assessment and Plan of Treatment class. The SDOH Assessment element of the same class comes fromclinical_observations.Two departures from spec File 15:
categoryis not a column.category:AssessPlanis1..1with a fixed pattern (careplan-category|assess-plan), so it is a constant — Payerbox sets it, as it already does fortext.status.period_endadded. The spec has onlyperiod_start.intentis pinned to 4.0.1: unversioned resolves to R5, which addsdirective, a code not valid under US Core 6.1.0.USCDI index table, rebuilt so each column links to its own authority:
All 34 profile URLs verified 200; class URLs read from the USCDI index rather than guessed.
social_historywas documented in patient-demographics but never linked — fixed.The index change rides here because row 15 links to the new care-plans page.
