Release OpenCode Loop 0.5.38

Release OpenCode Loop 0.5.38 with the Loto Diyarı long-session hardening merged in #153/#154 and the exact OpenCode 2.0.11 compatibility work merged in #156.

Release contents

• bump package + lock metadata to 0.5.38 and update the README stable marker;
• route terminal session.error into Loop run ownership, fail closed on non-retryable provider/auth/policy failures, and back off retryable API failures without consuming logical run budget;
• deduplicate stale/completed recovery diagnostics and pause repeated waiting-user steady states for ordinary unlimited continuation;
• preserve the OpenCode 1.x server contract while adding the OpenCode 2.x { id, setup } local-plugin contract required by 2.0.11;
• support the current 2.0.11 command/session field shapes and native event stream while retaining the older beta adapter path;
• include exact OpenCode 2.0.11 real-host coverage proving plugin activation, Loop command registration, two autonomous turns, control lifecycle mutation, and max-run stopping;
• keep Ubuntu/Windows CI, minimum/latest plugin compatibility, Goal coexistence, V2 lifecycle, real-host and bundle-parity gates on the release head.

This release branch was rebuilt on top of the post-#156 main head, so the published 0.5.38 package includes the dual ./server entry and generated bundle changes.
