Adds POST /workitems/{id}/delegate (optional operator note rendered into
the convergence brief) alongside the existing per-item commands, and two
auto-escalation conditions (audit max-iterations without passing, repeated
terminal failure after retry) behind CodeyBox:DelegationEscalation, default
off. Automatic escalation fires at most once per item, never after a failed
delegation turn, preserves the failure signal, counts triggers by condition
on codeybox.delegation.triggers, and competes through normal pickup order.
Tests: 61 new (endpoint, policy/counting, transition, metering, fairness,
sweep, pipeline integration incl. a full escalate-fail-retry-no-rearm loop).
Affected suites re-run with failure sets byte-identical to the clean-tree
baseline (pre-existing environmental git/sandbox failures only).

Replaces the process-global Log.Logger sink swap (plus Log.CloseAndFlush in Dispose, which tore down the shared logger while WebApplicationFactory hosts in sibling collections were still using it) with a dedicated logger pushed through AuditLog.PushScopedLogger. The AsyncLocal scope flows into the awaited sweep and is immune to concurrent global logger rebuilds, so the terminal_failure_classified assertion is deterministic and the class leaves the GlobalSerilog collection.
