Implements the gated trigger that spends a banked quota-reset credit on a shouldSpend=true advisor verdict: off-by-default flag, dry-run-by-default with a separate AllowLiveSpend decision, deterministic idempotency keys persisted before dispatch, persisted per-period cap (1 credit/$80 per 30d), per-attempt kill-switch, balance unknown/stale/empty refusals, reconcile- on-ambiguity instead of blind retry, serialized consume path, per-decision audit trail, and an injected transport so no test can reach the live endpoint.
